Revalidation Dates

To obtain the applicant's manual click on the following link (Download).

The total cost of the exams and re-exams will be $107.00 ($100.00 exam fee and $7.00 IVU).

You can request an exam by one of the following ways:

  • Go to www.revalidasonline.com and complete the registration, payment and test date assignment process.
  • Complete and print the test application form (available at www.revalidasonline.com), include a money order payable to Test Innovations and send to:

Notes

  • All Interested Applicants for Reasonable Accommodation must file an application using the forms established by Regulation No. 6463 of May 22, 2002.
  • Pursuant to Section 17 of Act No. 10 of April 26, 1994, page 9, "any applicant for renewal who has submitted his application with the supplementary documents accompanied or requested, before thirty (30) days prior to the expiration of his license, shall have his license automatically extended for the term to be taken by the Board in considering his application".
